Contractor License Bond - Online Application
Contractors License Bonds-please note that downloadable applications for

INSTRUCTIONS FOR APPLICATION
The numbers refer to sections on the applications above.

Check whether you want a one-, two-, three-, four-, or five-year bond.

Fill in this section completely. List your primary and then all secondary
classifications under License Class. For Date Bond to Take Effect, write in
date of your choice or leave blank and we will use the date the bond is
processed.

Most applicants want us to file the bond with the CSLB. (We will mail you a
confirming copy.) Check this box only if you want us to mail the bond to you so
you can mail it to the CSLB yourself. Send the original, not a copy, to the
CSLB.

Check the type of business you listed on your license application. You may not
change the type of business unless you file a new application.

SOLE OWNERSHIP:
The owner fills in the section. List the title as "Owner."

PARTNERSHIP:
Two authorized partners must fill in the section

CORPORATION: President and one other officer must sign for the
corporation and as individual indemnitors. If the corporation has only one
officer, a second indemnitor must fill in the Name, Social Security Number, and
Signature Sections.

Fill in your payment information. If paying by credit card, be sure to include
the billing address. If paying by check, make payable to "J&C Corbett
Insurance Services, Inc."
Remember to keep a copy of all paperwork for your records.
CHECKLIST: Be sure to include the following with this application.
IF YOU ARE APPLYING FOR AN ORIGINAL CONTRACTOR'S LICENSE:

A check payable to the "Contractors State License Board" for your initial
license fee.

A check payable to "J&C Corbett Insurance Services, Inc." for the bond
premium, or complete credit card information on front.

The "requirement" letter from the CSLB. (Received after you take your exam.)

A complete & signed copy of the answer sheet from the CSLB asbestos
open-book test.

A signed "Exemption from Worker's Compensation" form (if you have no
employees.)
